Service Redesign Projects - July 07 to March 08
Throughout July 2007 and March 2008, patient, carers and their representatives, health professionals and managers began their detailed discussions about what the future could look like if health organisations in Hartlepool, Stockton and parts of Easington and Sedgefield worked together to build a system which would be the envy of the country.
This was broken down into various projects:

The groups looked at the way services could be designed to make sure they are provided in or near people’s homes where possible, in GPs’ surgeries, in integrated care centres or in hospital.
Each of the groups looked at:
- The service philosophy - the things they thought vital to provide the very best care for patients
- The way health services work at present
- How demand for health services will change in the future
- Development of a working model for the future
- Service specification
- Recommendations for future development
The groups also looked at the best way to provide safe, effective, personalised and fair health care. This was broken down into different health areas:
- Public health prevention
- Primary and community care services
- Access
- Facilities
- Discharge, follow up and on-going care
